Club Basketball Teams in Manitoba must complete the form below annually and submit the appropriate payment prior to registering for a club league, showcase, or tournament.
NOTE: This form is for Manitoba-based club basketball teams only. Community Centre teams that participate in the Winnipeg Minor Basketball Association (WMBA), teams in the Westman Youth Basketball Association (WYBA) or Adult Teams (age 18+) are NOT to complete this form as the data is collected as part of your respective league registration.
